Transitional Care Inpatient Unit (TCU)

About

This 40 bed transitional unit cares for patients who have a firm discharge destination but face a temporary obstacle, such as awaiting home and community care services or slow stream rehabilitation and convalescent care. Focused, short term supports will enable these patients to be more quickly and safely discharged.

Patients are transferred to this unit one a discharge plan is in place. This unit does not accept direct admissions from RVH Emergency department.

Our healthcare team consists of physicians, registered nurses, registered practical nurses, patient care assistants, dietitians, pharmacists, physiotherapists, occupational therapists, a discharge planner, social worker, a respiratory therapist, a speech language pathologist, a chaplain, unit clerks, and volunteers. Each member has a specific role to play to ensure optimal health and seamless transitions. They strive to provide exceptional care to each patient and family.

Step-by-step Directions

Street: Gallie Court
Traveling on Georgian drive you will pass Georgian College and TURN RIGHT onto Gallie Court. After the stop sign, proceed FORWARD and TURN RIGHT into RVH’s Main road entrance.

You will see Rotary Place

Parking: Main
Proceed FORWARD through RVH’s Main road entrance. RVH’s Main Parking lot can be found ON YOUR RIGHT.

Enter RVH’s Main building entrance and FOLLOW the Blue Navigation Line to the Central (Green) elevator.

Take the Central (Green) elevator to LEVEL 4.

From the elevator TURN LEFT and proceed FORWARD through the hallway double doors. Passing the second Central (Green) elevator bank, Transitional Care Inpatient Unit is located at the end of the hallway ON YOUR LEFT.
