Advance Care Planning – a process of thinking about and sharing your wishes for future health and personal care. This website has resources to help you and your loved ones create an advance care plan.

North Simcoe Muskoka Palliative Care Network – an organization that links you to the right services and supports that you need locally. The Network has a Hospice Palliative Care Consultant team with special skills in pain and symptom management who can help address your needs.
