Your First Visit


The New Patient Clinic works with your referring physician
and Cancer Care Ontario to ensure you get the best
possible care.

People visit the clinic for many reasons. You will have
appointments with the Oncologist or General Practitioner in Oncology to plan treatment and for follow-up
after treatment. People also visit the clinic and/or hospital
for diagnostic testing, administration of chemotherapy,
transfusions of blood products, counseling and education.


What to Expect

At your first appointment, we will:

  Ask about your medical and surgical history
  Ask about the medications your are currently taking
  Perform a physical exam
  Talk about your treatment plan
  Give you education materials to meet your needs
Give you information on your upcoming appointments

What to Bring to Your Appointments

•  Ontario Health Card
•  Private drug insurance information
•  A list of your current medications, vitamins, and herbal supplements you are taking
•  A list of your allergies
•  A list of questions or concerns that you may have 
A family member or support person (optional)

    Insurance Forms  

    • If you have an insurance form or disability forms, etc., to be completed by your Oncologist, please bring them to the 2nd level Health Records Department for completion. Consent will be required to ensure that your physician  has your permission to provide medical information to insurance companies.  The forms can take up to 30 days to be completed. If you require the form sooner than that, please discuss this with the Health Records Team. You will be contacted by the Health Records Team once the form(s) are completed.
